About the Role You will work within the Lifestyle Team to nurture individual interests, maintain motivation and stimulation of residents through activities of interest and personal support. You will be responsible for maintaining accurate documentation of activities including progress note entries and evaluation of the effectiveness of care services delivered.
You will work across two locations:
Monday at Prague House, Tuesday-Friday at Berengarra
Classifications for this position will range from IN31 - IN33 ($55 510- $64 464 per annum) plus an additional overaward.
Your Contribution
Minimum Certificate III in Lifestyle/Leisure or equivalent
Intermediate computer skills using MS office and healthcare database programs
Previous experience working in a residential care setting or similar environment
Demonstrated knowledge and understanding of dementia management strategies and mental health
Ability to practice within a “person centred care” framework
Actively contribute to supporting a high functioning multidisciplinary team and be able to work independently
Assist and support change, and implement process improvements
Communicate effectively, with residents, representatives and staff at all levels
Current NDIS Worker Screening Check, or willingness to obtain

About Berengarra is a multi storey, 90 bed state of the art residential aged care facility, comprising of 60 mainstream and 30 Aged Persons’ Mental Health (APMH) beds designed to deliver contemporary best practice person centered care.
Prague House is a purpose built 45-bed facility that caters to a resident group with a background of homelessness or significant risk of homelessness, often accompanied by some of the following - a history of poly substance misuse, acquired brain injury, complex mental health diagnoses and disabling social isolation. Prague House welcomes people with a disability who are participants in the National Disability Insurance scheme.
Berengarra is co-located with St George’s Hospital in Kew, adjacent to Prague House - Cotham Rd, Kew
